LM358 weak-signal amplifier module

Note: There appears to be an error, either on the image or on the board itself. On the image, on the end connector closest to the two pots, V is shown as the right pin. However on the board I have, V is labeled on the left.
Also, in the other image, the two stacked pots are both labeled as "Zero," but one of them clearly must be 'Gain'.

Operation:
One end of the board has a 3-pin connector labeled V - D - G. The image annotation says the 'D' is a digital output. Presumably this goes active when the signal exceeds some value as set by the threshhold pot.
